Waiver: (Your Application Submission will be kept on file to represent that you agree to the terms of this waiver.) In conjunction with my participation in activities sponsored by The Miami Mavericks Tennis Club (referred to as The Club) and the resulting activities and events, I agree to waive any and all rights and claims for bodily or emotional injury, injury to my reputation, or any other damage or harms that I might suffer, or any other claims of any nature I may have against The Club, The Gay and Lesbian Tennis Alliance (GLTA), or any organization, facility, or individual associated with The Club. I understand, acknowledge and agree that I will be participating at my own risk, and the above organizations, facilities and individuals will not be responsible for any harm, injury or damages which I might suffer. I also waive any and all rights to claim damages or remuneration against The Club or any organization or individual associated there with which may arise as a result of reference to me or photographs thereof which may appear in any publication, media or the internet. Complete the remaining steps to apply for membership in The Miami Mavericks, to re-new your membership, and/or join activities. The cost of the activities will be calculated automatically. Please maintain a copy of your payment receipt that will be emailed to you by PayPal, just in case any discrepancies arise. You will be immediately added to the roster for the events you have selected. You will receive an E-mail from the appropriate event director confirming your addition to the roster. If you do not receive a roster confirmation email within 3 business days, please contact one of the Board of Directors to research your application entry. Thank you for your interest in joining MMTC.
